What is a Care Manager?

A Care Manager is a specially trained professional who can help individuals who have a physical or emotional challenge, no matter the person's age.

Care managers are experienced working with issues such as:

• long term care placements,
• advocacy in medical, mental health, medical or educational settings,
• relocation and changes in living situations of older adults,
• drug rehabilitation,
• eating disorders,
• psychiatric disorders,
• assistance with driving evaluations, and
• physical challenges that individuals face.

They have an extensive knowledge base about the availability, costs, and quality of services in the local community, so a client or their family member can make the most informed decision possible for themselves or their loved one.
