Care Process

Silver Moon gives high quality, personalized care based on the needs of each individual.

• An in-depth assessment is done on each client.
• A plan of care is developed from this assessment, and is customized for each person.
• Follow-up and monitoring of services will be provided to ensure continuing quality of care.

Assessment

The first meeting will involve getting essential information, along with a brief history of the person needing services.

Attendance of both the individual and caring family members is strongly recommended at this first meeting.

Further evaluation will be done based on how much information is gathered at the first meeting, and based on the situation of the individual needing services.

Plan of Care

After a complete evaluation is done, recommendations will be made on the basis of what is in the best interest of the care of the individual. This will be the plan of care that will be designed to meet the needs of this individual.

Follow-up and Monitoring

I will do a follow-up on each and every client to be sure that his or her needs are being met, and that the services provided are appropriate for the level of care of the individual.

I will monitor all clients that request and agree to ongoing monitoring of services. I suggest monitoring of services once they are in place as situations change, and I want to ensure the best care possible for each client I serve.
