Silver Moon provides help to families and their loved ones who face the complex issues of aging, maturity, and the life cycle changes that occur as a result. Silver Moon will assess each client, and help you and your family make informed decisions to get the best care possible, while maintaining the dignity and respect of your loved one.

Silver Moon provides advocacy for persons of any age. We help those with physical, emotional, or educational/learning challenges.

What services does Silver Moon provide?

Here's a full list of typical services provided compiled from experience with our many past clients:

  • Advocate in educational, medical and mental health settings
  • Arrange and monitor in-home care services
  • Assist with placement at a long term care facility
  • Help with financial matters, such as Medicare, Medicaid, and referral to elder care attorneys as needed
  • Counseling for families and loved ones as they face life cycle transitions
  • Advocacy for loved one in long term care facility
  • Services as liaison between family members, client, and other involved services; especially when family members live at a distance
  • Assistance with relocation from one living situation to another
  • Crisis intervention
  • Evaluation of eligibility for services and assistance
  • Appropriate referrals for medical care, and other needed services
  • Bill paying assistance
  • Transportation for non-emergency services such as doctor's appointments, and grocery shopping
  • Coordination of medical care with all doctors and caregivers
  • Respite care
  • Hospice care
  • Follow up and monitoring of all services provided- this is essential
  • Excellence in care with exceptional ethics and values in place to keep the client the foremost focus of the care
  • Assist with addressing driving issues

Coverage Area

Silver Moon covers South Central Indiana including Indianapolis and surrounding counties. Will travel according to need of clients in other areas by personal arrangement

Costs

Costs are dependent on level of care, and are not covered by Medicare, Medicaid, or any other type of insurances.

Silver Moon has very reasonable pricing compared to other providers in the area. Fees are based on a competitive hourly rate, and the total amount for services varies upon the individual needs of each client. Since each situation is different, let us know more about your situation so we can discuss pricing for your needs.
